Eligibility and Assessment

 

The Aged Care Assessment Team (ACAT) assesses eligibility for a home care package. You will be allocated a package based on your care requirements if approved. Assist Lifestyle’s team can help guide you through this process, ensuring you get the support you need.

Client (2-Step) Sign-Up Process

Step 1: Complete the Online Questionnaire & Sign the Home Care Agreement

Start by completing a brief online questionnaire that helps us understand your care needs. This questionnaire includes essential information, such as a health summary, medical details, and any preferences for your care. At the end of the questionnaire, you’ll be prompted to review and sign our Home Care Agreement, which formalises your enrollment in the program.

Step 2: Attend a Care Plan Meeting

Once the questionnaire and agreement are submitted, our team will contact your nominated representative to schedule a care planning meeting. During this phone meeting with an intake assessment officer, we’ll work together to design a personalised care plan and budget based on your needs, ensuring every aspect of your Home Care Package is optimised to support your independence.

How to Apply for a Home Care Package

Applying for a Home Care Package may seem complicated, but we’re here to easily guide you through the process. Here’s a simple step-by-step guide:

 

Step 1: Check Your Eligibility

You can apply for an ACAT assessment through My Aged Care, either online or over the phone.

Step 2: Complete Your ACAT Assessment

An ACAT assessor will review your situation to determine the level of care you need. This assessment can be done at home or over the phone.

 

Step 3: Receive Your Assessment Outcome

Step 4: Package Assignment and Referral Code
