Individualised Living Options (ILO)

Empowering Individuals With Tailored Living Options to Enhance Independence and Achieve Their Unique Goals

Individualised Living Options

What Does Individualised Living Options Support Mean?

An Individualised Living Option (ILO) is an NDIS-funded support package that helps you live in the home environment of your choosing in a way that best suits your needs and preferences—it’s not the actual home itself. Instead, ILO funding enables you to design a tailored mix of both NDIS-funded supports and informal supports (family, friends, neighbours, or community networks) to help you live safely and independently.

Key features of ILO support include:

Choice and Control Over Your Home Environment

  • You decide whether to live:
    – With housemates in a shared residence,
    – In the home of a host carer, or
    – In a private rental or family-owned property.

  • The ILO package is structured around your unique goals, preferences, and living arrangements—rather than fitting you into a pre-set group home.

Two-Stage Funding Process

  • Design and Exploration
    – Work with Assist Lifestyle to explore where you want to live, who you want to live with, and what supports you’ll need.
    – Collaborate on developing an ILO plan that outlines support roles, responsibilities, and expected costs.

Combination of NDIS-Funded and Informal Supports

  • NDIS-Funded Supports may include:
    – NDIS assistance for decision-making (choosing and/or setting up your home)
    – Personal care, household management, or skill-building to increase independence
    – Support to build and maintain social connections and community engagement
    – Assistance with managing emotions or behaviours within your home setting
    – On-call or emergency backup supports if needed
    – Training and mentorship for the people who support you

  • Informal Supports may include:
    – Assistance from family, friends, or neighbours with daily tasks
    – Contributions from community volunteers or self-organised helpers
    – Support from shared-living housemates or host carers who assist on a regular basis

Implementation and Review

  • Once your design is approved in your NDIS plan, we help you put it into action—recruiting support workers, coordinating informal supports, and setting up systems for day-to-day living.

  • Your ILO can be reviewed and adjusted over time to match any changes in your goals, health, or personal circumstances.

What ILO Does Not Cover

  • Your Rent or Mortgage — ILO funding does not directly pay for rent, mortgage repayments, or bond payments.

  • Everyday Living Costs — groceries, utilities, phone/internet bills, or general household items (e.g., furniture, bedding).

  • Other NDIS Home & Living Supports — such as Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), or funded home modifications (these are funded separately if you are eligible).

  • Activities Outside the Home — ILO does not include supports for work, study, sports, or community activities—these will be covered under other parts of your NDIS plan if required.

  • Funding Informal Supports — Payments to family or friends who assist you are not considered reasonable NDIS-funded supports.

Because ILO is focused on your living situation, we work closely with you to ensure all the supports in your plan meet NDIS funding criteria and align with your home-and-living goals.

What We Offer Under Individualised Living Options Support on Sunshine Coast

At Assist Lifestyle, we guide you through every step of creating a successful Individualised Living Options on Sunshine Coast by providing:

  • Design & Exploration Phase

    In-Depth Needs Assessment

    • We spend time with you (one-on-one interviews, home visits, or virtual consultations) to understand:
      • Your desired level of independence and privacy
      • Preferred location (e.g., neighborhood, proximity to services)
      • Desired living arrangement (shared house, host home, private rental)
      • Any accessibility or assistive technology requirements (e.g., grab rails, step-free access)

    Support Mapping & Budget Planning

    • We identify which tasks can be managed by informal supports (family, friends) and which require NDIS funding.
    • We outline realistic cost estimates for NDIS-funded supports (e.g., number of support worker hours per week, on-call backup, training costs) so that your ILO proposal fits within your NDIS budget.

    Person-Centred ILO Plan Development

    • Based on your goals and preferences, we co-design an ILO blueprint that details:
      • Where you will live (address or housing type)
      • Who you will live with (housemates, host carer, or solo)
      • What tasks you want NDIS-funded supports to cover (personal care, meal prep, home management, skills development, or emotional/behavioural coaching)
      • How informal supports will complement NDIS-funded supports

We prepare documentation to submit to the NDIS—complete with justification, cost breakdowns, and expected outcomes.

  • Implementation & Support Mobilisation Phase

    Provider Identification & Recruitment

    • We assist you in recruiting and onboarding qualified support workers (or host carers) who are trained in:
      • Delivering personal care and household assistance in home environments
      • Capacity-building techniques (teaching you skills rather than doing tasks for you)
      • Supporting your social and emotional wellbeing

    • We organize any necessary training or orientation for informal supports (family, friends, neighbours), ensuring everyone understands your ILO goals and safety protocols.

    Setting Up Your Home Environment

    • If you need adaptive equipment or home modifications (e.g., ramps, lever taps), we liaise with allied health professionals (OTs, physiotherapists) to determine requirements.
    • We support you to establish communication systems (e.g., on-call phone, emergency contacts) so help is available when needed.

    On-Going Service Coordination

    • We create a schedule for NDIS-funded support worker visits (weekly check-ins, meal prep assistance, personal care) and coordinate on-call back-up for nights, weekends, or emergencies.
    • We remain connected with informal supports—offering guidance on boundaries, handovers, and how to escalate issues if challenges arise.

    Regular Review & Adjustment

    • We set milestone review dates (e.g., after 3 months, 6 months) to evaluate:
      • Your level of satisfaction and independence
      • Whether you need more or fewer NDIS-funded hours
      • Any changes to your health, routines, or preferences that require modifications to your ILO supports

    • If you decide to move house, change housemates, or alter your support mix, we assist with the paperwork and budget adjustments in your next plan review cycle.
Why Choose Us?

Why Choose Ilo Support From Assist Lifestyle?

Expertise in ILO Design & Implementation

We are creating tailored ILO supports on Sunshine Coast, QLD. Our team has extensive experience guiding participants through the ILO process—from initial design proposals to coordinating multidisciplinary allied health assessments and liaising with the NDIA on funding approvals. We stay current on NDIS policy updates, ensuring your ILO plan meets all eligibility and funding criteria.

Person-Centred, Strengths-Based Approach

We listen to your vision for independent living and focus on leveraging your skills, talents, and informal support networks. Rather than imposing a one-size-fits-all solution, we co-design a living arrangement that reflects your unique goals—whether that’s increased privacy, community connection, or skill-building.

Holistic Coordination Across Supports

Because ILO involves both formal NDIS-funded supports and informal supports, we act as the central hub—coordinating with:

  • Support workers and host carers (training, schedules, back-up planning)
  • Family, friends, or neighbours (onboarding, communication protocols)
  • Allied health professionals (OTs, physiotherapists, behaviour specialists) for home setup and capacity building
  • NDIA planners and Local Area Coordinators to secure necessary funding and approvals

Focus on Safety, Stability, and Flexibility

  • We conduct comprehensive home safety assessments—identifying potential hazards and recommending adaptive equipment to reduce falls or injuries.

  • Our ILO support is designed to be flexible:
    – If your needs change (e.g., health fluctuations, changes in informal support availability), we can reallocate hours or adjust the support mix.
    – If you decide to move home or change housemates, we handle the transition smoothly—updating budgets, modifying rosters, and re-securing funding as needed.

Commitment to Long-Term Outcomes

  • Beyond establishing your initial ILO setup, we track key indicators of success:
    – Your level of independence in daily tasks (personal care, meal preparation, household management)
    – Your satisfaction with social connections and emotional well-being (via regular check-ins)
    – Your ability to make informed decisions about your life and home environment

  • We advocate on your behalf when new supports are required—such as additional capacity-building, behavioural support, or home modifications—so you can continue to thrive in your chosen home setting.
