What is NDIS Support Coordination?

Support Coordination is a capacity building support that helps you make the most of your NDIS plan by:

Levels of Support Coordination

There are three levels of Support Coordination that may be included in your NDIS plan, depending on your needs and circumstances:

1. Support Connection

This level focuses on helping you build the skills to connect with informal supports, community services, and NDIS providers. The goal is to help you gain confidence and develop the ability to manage your own supports over time.

2. Support Coordination

This level provides support to help you understand and use your NDIS plan effectively. A Support Coordinator works with you to organise and manage a mix of supports, helping you build independence, maintain service relationships, and stay connected to your community.

3. Specialist Support Coordination

This level is designed for participants with more complex needs. A Specialist Support Coordinator helps manage challenges in your support environment, coordinates multiple services, and ensures your supports are delivered consistently and effectively.
